Fees and Insurance

We accept insurance from the following health plans:

Self-Pay/Out-of-Network Fees

All fees are secured with your credit card and will be collected the morning of your appointment. Credit card information will be collected prior to your appointment and must be kept on file.

Co-pays

Your insurance copay will depend on your plan and whether you have met your deductible for the year. Contact your insurance provider if you have questions about your particular situation. Copays are due the day of the appointment and are taken electronically in the morning.  

Emotional Support Animal Assessment and Paperwork

Note this is not covered by insurance, is done outside of appointment time and is offered only to our current patients.

Initial: $200.00 

Updates: $100.00

Missed appointments and late cancellation fees

In order to avoid a missed appointment fee, we require greater than 24 hours notice to cancel appointments. You are financially responsible for missed appointments and late cancellations. The fee is $100. This cost is not covered by insurance. Patients must keep an active credit card on file for any fees and for copays. 

Services outside of appointment time

All refills, medication changes and completion of forms and paperwork should be completed during scheduled follow-up appointments to avoid fees for services outside of appointment times. Fees for work completed in-between appointments are not covered by insurance. This time does not include calls to let the doctor know you are doing worse or having difficulty with your medication.

Phone calls/emails/texts/forms greater than 10 minutes time:

  • 10-15 minutes - $150

  • 16-30 minutes - $300

  • 31-60 minutes - $450

We encourage every patient to call their insurance prior to their appointment and verify eligibility and benefits for “Mental Health.” Also verify applicable deductible (how much has been met), coinsurance and copayments. Patients are responsible for co-payments, deductibles and any amount not covered by their insurance. Patients are responsible for keeping an active credit card on file which may be used to collect patient responsibility balances due past 30 days.
